国境なき保健サービス：世界で最も危険な移動ルートで女性を支援する

国際移住者デーに際し、南米コロンビアと中米パナマを結ぶ危険な移動ルート、「ダリエン地峡(Darién Gap)」を訪れたヴァレリー・ドゥルダン(Valerie Dourdin) IPPF人道支援部長より現地の状況とIPPFの活動について報告します。   「移動する人々」 「移動する人々」とは、紛争、暴力、経済的苦境、気候変動などの理由から、半ば強制的に本来の住居地を離れて移動する人（移民）を指します。移動する人々の多くが、母国での選択肢を全て失い、安全、機会、生存のために出国を余儀なくされた人々です。このような避難の旅は非常に危険で、数週間、数カ月、あるいは数年かかることもあります。身分証明書がない移民は非正規ルートで複数の国を横断しなければならず、途中で暴力や搾取の被害にあったり、人身取引に巻き込まれる危険にも直面します。 現在、世界では大きく分けて二つの主要な移動ルートがあります。一つ目はアフリカ大陸を縦断するルートで、移民は地中海を目指して北へと移動します。二つ目はアメリカ大陸のルートで、多くはチリ、アルゼンチン、ハイチから始まり、コロンビアや中米を北上してアメリカを目指します。近年では、アフリカや遠く中国からも南米に渡り、このルートを進む人々が増えています。そしてこのルートで特に危険な通過地点となっているのが「ダリエン地峡」です。コロンビアとパナマの間に広がるおよそ70マイル（約113キロメートル）の密林地帯で、極めて過酷なルートとなっています。 女性やその家族は移動に際して大きな苦難を強いられていますが、彼女たちが受ける暴力や搾取、生き抜く手段について語られることはほとんどありません。   ダリエン地峡を渡るということ ダリエン地峡は、アメリカ大陸の移動ルートの中で最も危険な国境越えのひとつであり、一帯を武装犯罪組織が支配しています。旅をする家族にとっては、悪夢のような道のりです。川は突然増水し、深い森は迷いやすく、多くの人々が忽然と姿を消します。健康な若者なら5日で渡れるかもしれませんが、小さな子を連れた母親だと10日から15日以上かかることもあります。 2022年に訪れたときは、このルートを利用する人が増え始めていました。2023年には約60万人に上り、2024年は約23万人に急落しましたが、私が強く感じたのは、人々の切迫感が増していることです。十分な資金や情報を持たない低所得の家族が増え、その多くが密航業者に全財産をだまし取られています。女性たちは「戻る場所がないから、前に進むしかない」と語っていました。生まれたばかりの赤ん坊を抱いて移動を始めようとする母親や、ビーチに行くと思っている幼い子どももいました。どの親も子を守るために十分な備えができているとは言えない状況です。 2022年の時点では、地峡のジャングルには入ることさえできませんでした。現在はけもの道のようなものがあるものの、ぬかるみがひどく進むのに時間がかかり、荷物を持っての移動は困難なため、大抵の家族は終着地のパナマに到達する頃には身一つになっています。報告によると、このルートを渡る女性のおよそ3人に1人が性暴力を経験し、半数以上が強盗の被害に遭っています。   IPPFコロンビアの活動 IPPFコロンビアは、50以上のクリニックを運営し、数十年間に渡ってセクシュアル・リプロダクティブ・ヘルス(SRH)サービスをコロンビアで提供してきました。危機的状況にも対応し、その活動は高く評価されています。多くの移民はダリエン地峡に入る前にネコクリやカプルガナなどの町に立ち寄ります。私たちが検討したのは、その地域でのIPPFのモバイルクリニックのサービスをさらに充実させることです。クリニックでは、家族計画、HIV検査、性感染症(STIs)検査などのSRHサービスを提供しています。ダリエン地峡の終着地であるパナマでは、地峡に入る前と通り抜けた後の両地点で女性が支援を受けられるよう、SGBVを中心にSRHケアの連携ネットワークの構築について、地元団体や政府の組織と話し合いました。
